NEW PATIENTS

Your first visit to Dr. David Bastawros establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Patient Forms
In order to expedite your registration with the practice and to follow compliance regulations with Red Flag rules, please bring the following documentation with you to your first visit (no exceptions):

  1. Insurance Card (including Medicare Card).
  2. Photo Identification (driver's license or state I.D).
  3. New Patient forms (11 pages).
  4. Complete List of Medications (including dosage & interval).

Patient Instructions
It is very important that you arrive either early or promptly at the scheduled time of your appointment with all your forms completed and all required documentation. This gives us time to complete all the necessary registration paperwork prior to seeing the doctor. If you do arrive greater than 5 minutes late, please understand that your appointment most likely will be rescheduled.

If your insurance policy (i.e. HMO, POS plans) requires a referral from your primary care physician, it is your responsibility to obtain this required referral prior to your initial visit.

What to expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site.
