Please bring the following documentation with you to every visit (no exceptions)L
Insurance Card (including Medicare Card).
Photo Identification (driver's license or state I.D).
If you are unable to complete your registration online, please arrive at least 30 minutes prior to your scheduled appointment to complete our registration process including printing and completing the additional documentation:
New Patient forms (10 pages).
Complete List of Medications (including dosage & interval).

It is very important that you arrive either early or promptly at the scheduled time of your appointment with all your forms completed and all required documentation. This gives us time to complete all the necessary registration paperwork prior to seeing the doctor. If you do arrive greater than 5 minutes late, please understand that your appointment most likely will be rescheduled.
As this is a specialist office, it is the office policy to treat one podiatric condition at a time to ensure full evaluation and treatment of that condition. The staff will make any further and future appointments to address multiple conditions.
If your insurance policy (i.e. HMO, POS plans) requires a referral from your primary care physician, it is your responsibility to obtain this required referral prior to your initial visit.
