LIMB PRESERVATION CENTER
Dr. David S. Bastawros, DPM is affiliated with Baylor Scott & White Health, specifically with The Heart Hospital Baylor Plano, and works full-time at the Center for Advanced Cardiovascular Care™ (CACC) located in Medical Pavilion 2, Suite 300, on the Baylor Plano campus. Dr. Bastawros co-founded the Limb Preservation Center program at the CACC. This program primarily focuses on the comprehensive treatment of the diabetic, vascular, and neuropathic foot with all of its complications. Services available include education for newly diagnosed diabetics, comprehensive diabetic foot exams, treatment of podiatric injuries, evaluation of persistent diabetic foot pain, wound care, and advanced limb salvage, just to name a few.
This is not a physician's or podiatrist's office setting - this is an advanced hospital outpatient multi-disciplinary center where physicians and surgeons of many specialities function to provide advanced treatments for the medically complex patient.

This is an outpatient hospital department, and physician referral is required. Please have your treating physician fax a written consultation request for Dr. Bastawros and have them include the following:
- Past medical history
- Past surgical history
- Current list of medications
- Family history
- Social history
- Risk factors

Any patients who wish to obtain their current medical records can access their patient chart through our Patient Portal, which will stay active until July 31, 2014. If you currently do not have access to the Patient Portal, please contact the office at (972) 964-3668 to set up your access. If you chose not to use the Patient Portal, any medical records requests will need to be done in writing and faxed to (972) 964-4810 with your authorized signature, and will incur a $25 fee which must be received with this written request – your request will not begin processing until payment is received. These requests may take up to 30 days.
